   I give permission for my child to attend all scheduled NextGen activities held on the St Pauls Church property
   I understand all NextGen leaders and helpers have completed a Safe Ministry course provided by Anglican Diocese.
   I understand that leaders and helpers over the age of 18 years have a cleared Working With Children number, allowing them to care for and interact with children.

I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behavior may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of NextGen Ministries.

I will provide the leaders with any information relevant to the wellbeing of my child prior to him or her attending an activity.
I confirm that this information given in this form is true and correct, and will advise NextGen leaders of any changes to this information.

YOUTH SPECIFIC
I give permission for my child to travel in a car driven by an approved Youth leader who hold a Provisional P2 Licence (Green Ps) or Full Licence, unless I advise the leaders otherwise. Parent's consent will be given for non-emergency situations.

I understand that some NextGen activities (more particularly for Youth Ministries) may involve the surrounding streets and parks in the Chatswood area in the company of NextGen Leaders.

SOCIAL MEDIA / PHOTOGRAPHY RELEASE

I give permission for photos / videos of my child to be used for promotional purposes including St Paul's Church website and social media. (We may take one photo of your child for our communications/advertising team to ensure no incidental photos taken by St Paul's authorised people is circulated on our social media and website platforms.)
